Book an Appointment
Urgent Appointments
If you require a Chaperone and/or a Translator for your appointment, please state this on the submission or on arrival to the surgery.
We understand that your need for an appointment may be urgent, eg needing same-day advice. This can be requested in the following ways:

- Telephone: If you can't complete the form online, please call 0208 693 2727 for assistance. This may take longer than submitting it yourself.
- In Person: Alternatively, you can attend the practice in person.

Routine Appointments
If you require a Chaperone and/or a Translator for your appointment, please state this on the submission or on arrival to the surgery.
We currently offer appointments at the surgery and the Tessa Jowell Centre (extended primary care service). Appointment options include both face-to-face and telephone/video appointments, and can be booked up to 7 days in advance. The type of appointment required will be reviewed daily by the on-site doctor.

- In Person: Alternatively, you can attend the practice in person.
Our trained staff will ask you questions about your request to ensure we can provide the appropriate treatment. Without this information, we may not be able to manage your request effectively.
Home Visits
If you need a home visit, you can help us by calling reception before 11am or by completing an online request.

Home visits are available for housebound or frail patients. Unfortunately, lack of available transport is not a valid reason to request a home visit.
The on-site doctor will review the request and determine the urgency of the visit required. Please be prepared to provide suitable details to enable the doctor to schedule house calls.
Blood Tests
Some blood tests can be requested without an appointment, eg drug monitoring, chronic disease management, specialist advised, etc.
Please specify:
- The exact blood tests required
- The reason
- The name of the Specialist team requesting the bloods (if appropriate, with documentation provided if not done so already)

Change or Cancel an Appointment
Please give us as much notice as possible so we can offer your appointment to someone else.
To cancel your appointment:
- Phone us on 0208 693 2727 as early as possible. Cancellations less than 30 minutes before your appointment will be marked as "Did Not Attend" (DNA).
DNA Policy
Missed appointments without a valid reason will be recorded, and repeated occurrences may lead to removal from our practice list.
Out of Hours
Enhanced Access
Ask at reception for enhanced access information
Life Threatening
Call 999 or go to A&E now if:
- you or someone you know needs immediate help
- you have seriously harmed yourself – for example, by taking a drug overdose
A mental health emergency should be taken as seriously as a medical emergency.
Urgent But Not Life Threatening
Visit an urgent care centre if:
- You have an urgent medical issue requiring on the day attention
Non-urgent
Use NHS 111 if:
- You need help now, but it’s not an emergency
There will be someone to provide you with advice and to direct you to a clinician if it is necessary.
